| Family Case
Management is a statewide program funded by the Department of Human services.
The goals are to reduce the rate of infant mortality, promote wellness and healthy
lifestyles in pregnant women and children up to the age of 2 years.
Case Management services include: supportive personal contact, health education, assessment of needs, referral to other social service agencies and medical services. Free pregnancy testing is offered during clinic hours only. If a pregnant woman qualifies, we will complete an application for a temporary medical card. She will then be referred to the Department of Human Services for ongoing medical assistance or we will assist her in completing a Kid Care application.
